Our Healthy Together Manager, Chloe, was shortlisted for the Inspirational Leader Award at the Norfolk and Suffolk Health and Care Awards 2026, and we were delighted to see her named as runner‑up at the ceremony in Newmarket. This recognition reflects her exceptional leadership and the transformative impact she has made for people living with Severe Mental Illness (SMI) across Suffolk.
Chloe’s strategic vision has reshaped how people with SMI are supported to improve their physical health and wellbeing. Under her leadership, the Healthy Together Team now supports 3,785 individuals, addressing the stark inequality that people with SMI experience, including a 15 to 20‑year reduction in life expectancy.
She leads with authenticity, compassion and lived experience. Chloe has created a psychologically safe and inclusive team culture where personal experience of mental health challenges is recognised as a professional strength. This approach fosters trust and enables meaningful engagement with service users, particularly those from deprived communities, who make up 64% of referrals.
The Healthy Together Team supports around 270 referrals each year, with 92% achieving their health goals within six months across more than 70 care pathways. Feedback consistently highlights the dignity, compassion and trust the team provides.
Chloe’s leadership has delivered measurable improvements in health outcomes and established a scalable, compassionate model of care for those most in need.
Being named runner‑up at the Norfolk and Suffolk Health and Care Awards 2026 is a well‑deserved recognition of Chloe’s dedication, innovation and unwavering commitment to improving the lives of people living with SMI. We are incredibly proud of her achievements and grateful for the impact she continues to make across Suffolk.
